H&C Care receives sensitive medical and personal information only for case preparation, membership consultation, and coordination support.
Why we use it
To organize the case, assess whether coordination is practical, communicate with relevant teams, and manage follow-up reminders.
Who may receive it
Relevant hospitals, doctors, nurses, administrative coordinators, translators, or case support partners involved in the requested pathway.
Your rights
Patients or authorized family members may request access, correction, or deletion where practical by contacting us by email.
Email and WhatsApp may not be suitable for emergencies or highly time-sensitive clinical instructions. Do not use this website for emergency care.
My Case
Only provide information needed for coordination. Do not upload passports, payment card details or unrelated records. The account owner and authorised staff can access the records; files are not public. This Supabase project's primary database and record storage region is Singapore; account and record data may be processed outside your country. Contact support@hccareadvisory.com to request access, correction or deletion. This is not an emergency or medical advice channel.
I am the patient or have their permission to submit these health records. I consent to H&C Care collecting and storing them for case coordination and communication. Sharing with a medical team requires separate authorisation.
